Introduction
The COVID-19 pandemic has completely disrupted the healthcare system imposing new challenges and providing a high degree of changes on the nursing practice and delivery of care. As responses to evolving clinical policies to unsustainable patient influx, the crisis also revealed structural weaknesses that nursing systems have always had, even as it produced innovation, resilience, and problem-solving that had been sorely needed. In this paper I will discuss the effects of the pandemic on nursing in all scopes of nursing practice and examine the responses by nurse leaders, staff nurses, and advanced practice nurses (APNs) through various decision-making methods.
Three models of decision-making, namely, Shared Decision-Making, Clinical Judgment Model, and Transformational Leadership, are compared to learn how they could be used to achieve effective responses during the time of crisis. In the end, the paper will present the best model and then suggest a funding plan to support implementation.
According to the background and significance of the issue, there has been an increase in the number of cases of childbirth in countries around the globe. This is because childbirth is an important issue in every country or society.
The COVID-19 pandemic affected practically all aspects of health care provision. Nurses encountered personal protective equipment (PPE) shortages of all kinds, high patient acuity, and moral distress to resource allocation and patient outcomes.
The International Council of Nurses (2021) ascertained the world nursing sector was affected by severe burnout, turnover and emotional trauma. Bedside nurses carried the anxiety of accelerating changes in care requirements, whereas nurse leaders and APNs had to contend with the planning at a system level, coordination of staffing and risk reduction.
As the research by Fernandez et al. (2020) suggests, a thorough literature review has shown the growing popularity of evidence-based decision-making and interprofessional collaboration. The importance of the issue is related to the depth of the demands directed to the sphere of nursing at all levels of these demands, which need quick, ethical, and effective decision-making procedures. The solution to this problem should include an assessment of such skilled decision-making models that promote clinical flexibility, independent responsibility, and shared problem identification.
Review of Literature and Theory base
Liu et al. (2020) and Shanafelt et al. (2020) research define such important themes as the decision-making under pressure, the responsiveness of the leadership, and moral resilience in nursing during a pandemic. Three models of decision-making turn out to be of specifically great importance:
1. Shared Decision-Making (SDM): In this model, much focus is placed on collaborative decision-making in which nurses, patients, and interdisciplinary groups participate in both clinical and organizational-level decision-making.
2. The Clinical Judgment Model (Tanner, 2006): Orienting to the situation assessment, real-time thinking, the model is used to support the front-line decisions of nursing on the necessity of a patient and the consideration of the context.
3. Transformational Leadership: This strategy is used to stimulate ideas and change by being a leader that is vision-oriented and empowering nurses and adapting the system-wide.
All these models are helpful towards various aspects of pandemic nursing, including bedside nursing and executive leadership.
Utilization of Decision-Making Models in the Various Levels of Nursing
Empowerment of Staff Nurse and Shared Decision-Making
The COVID-19 situation prompted the need of Shared Decision-Making (SDM) in the bedside in a hurry, because during the time, nurses needed to quickly adapt to new safety rules and treatment regimens. The participation of staff in the choice of infection control steps and patient deployment contributed to increased compliance and morale (Thomas et al., 2020). SDM enabled bedside nurses to correspond and suggest actionable changes in the operations. The model imposed transparency and a feeling of agency because all the decisions could not be made collaboratively in the environment of a crisis.
Clinical Judgment Model on Rapid Bedside Assessment
The model of Clinical Judgment was very essential to the staff nurses involved with unpredictable clinical deterioration of patients of COVID -19. Caprara et al. (2021) state that nurses used critical thinking, previous experience, and real-time decision-making to make rapid decisions related to escalating oxygen or providing emergency treatment. This model was also relevant to nurse practitioners (NPs) in the outpatient care, since symptom screening and triage was required to be accurate and efficient to prevent exposure and spreading.
Role of APNs and Nurse Managers and the Transformational Leadership
Nurse leaders and Advanced Practice Nurses were critical in the adoption of hospital-wide responses. Transformational leadership created resilience and vision when it came to uncertainty. Executives of nurses introduced telemedicine, the reorganization of the personnel system, and lobbying the safety of nurses at the policy level (Barrientos-Trigo et al., 2021). Leadership with authority enabled their teams to stay in communication, act as role models and stimulate professional growth in spite of the pandemic.
A comparative discussion of the model of decision-making.
The three models possess certain strengths and weaknesses with regard to the level of nursing practice. Shared Decision-Making helps to build unity and inclusion, although in the case of a crisis, when the immediate decisions must be made, it can be slow. Clinical Judgment Model works very well in quick bed-side decision making but might lack organizational vision. Transformational Leadership enhances systemic change and motivation of staff but it can be unavailable to frontline staff unless it is structured.
After comparing that side by side, Transformational Leadership is the most holistic strategy in the pandemic. It ensures combination of vision and responsiveness, promotes autonomy and enables change on micro and macro levels. Therefore, it provides the most suitable model of solving intricate difficulties such as pandemics.
Model of Practice that I Prioritized: Transformational Leadership
In the context of COVID-19, Transformational Leadership can help the nursing practice to go through the crisis most effectively. It aids in psychological well being of nurses, encourages innovation, and is institutional-goal-congruent. Leaders in the nursing field who applied this model motivated teams through integrity, communication, and common purpose, which played a critical role when the state of uncertainty lasted long. The other two approaches of Assisting Clinical Judgment at the bedside and promoting SDM in managerial decision-making can also be incorporated in this model to produce a flexible and dynamic care setting.
Proposed Funding Source of IM
In order to facilitate the application of Transformational Leadership practices, the Health Resources and Services Administration (HRSA) could be used as it provides grant services to help reinforce nursing leadership, workforce retention, and education. As an illustration of this, the NEPQR grant program targets an improvement of nursing education and patient care models. Alternatively, Sigma Theta Tau International has smaller research grants as well as implementation grants, which can do the same, involve training nurses on leaderships at different levels. The development of proposals and implementation thereof could be handled by nurse educators or DNPs to ensure that the recommendation is evidence-based and within the scope of nursing as a profession.
Conclusion
The COVID-19 pandemic revealed weaknesses of the healthcare system and demonstrated the strength and flexibility of the nurses. The insight into the patterns of decision-making in different nursing positions is also crucial to future crises planning and care delivery. Although Shared Decision-Making and the Clinical Judgment Model can be rather useful, Transformational Leadership is yet a general and flexible framework that will cover the needs of the staff, the patients, and the institutions. The future of nursing practice can be preserved by putting resources into this leadership style supported by relevant funding to ensure the survival of the field amid current and new changes.
